New Patient Registration

consultWe accept patients living in the village and surrounding area, including Woodhurst, Pidley, Fenton, Bury and Upwood. You can check if you are able to register with us by using the catchment area tab at the top of the page.

Patients registering at the practice will  not be assigned to any particularly doctor. We would, however, recommend as far as possible that you see the same doctor if you are being treated for an ongoing condition.

As you can appreciate it can take a little time completing the necessary paperwork, and we would therefore request that any patient moving to the area wishing to register with the Practice, does so prior to making an appointment with a doctor or nurse (preferably outside surgery hours). This avoids causing additional delays at the reception desk during a busy surgery.

What you will need to bring…

Ideally you should bring your NHS card, issued either at birth or when you last registered with a doctor . If you cannot find this card we have registration forms at the surgery.

Additional information…

Once you have registered the receptionist will offer an appointment with one of our nurses for a New Patient Registration Medical. This allows us to introduce ourselves to new patients as well as recording any past medical history. It is an opportunity to record any on-going medication needs. These medicals only take about 15-20 minutes and will include checking a urine sample.

The receptionist will endeavour to make this appointment at a convenient time to suit the patient - note early morning, lunchtime and early evening appointments available.

If you wish to register for online patient access, which allows you to book an appointment for a doctor or our nurse practitioner online, please contact reception who will be happy to print off the relevant details for you.

What if you move out of our catchment area...

If you move outside our catchment area you will be asked to re-register at a practice closer to your residence, for your own safety and best possible care. Upon notification of your new address you will be given 6 weeks to move to a new practice. After this time, if you are still a patient at the Moat House Surgery, your records will be sent back to the Health Authority and you will receive notification. From this date you will no longer be a patient at the surgery.

With effect from 1 July 2016 our practice boundary will be reducing.

If you are currently patients registered at the Moat House Surgery who lives in Ramsey or Somersham you will be able to stay registered, if you wish. However, if you move house and your new property falls outside our new practice boundary (please see the catchment area section on the website) you will need to re-register with a practice that covers your new property.

If you return to an old address (for example to stay with family), either on a temporary basis or as a permanent resident, that now falls outside our practice area, you will not be able to register at the Moat House Surgery.
