Moat House Surgery would like to invite you to join DoctorLink.
We have been looking at how we can improve access to Practice services; DoctorLink is our new online service where you can access advice and appointments on line.
Registration is simple and you can register here:
For all urgent appointments, we ask that you undertake a symptom assessment through DoctorLink.
The symptom checker will ask you a number of questions to help to identify your problem.
The outcome of your assessment will direct you to the right treatment, whether emergency care, your GP, nurse or self care.
If your outcome indicates that an appointment is required, you simply click on the request appointment link.
Once the request has been submitted, you will then be contacted by the Practice Team, to arrange an appointment within the identified timeframe (as outlined in the outcome of your symptom checker).
The information you provide will be shared with our clinical team (where appropriate) and will assist in providing robust information of your problem to the required clinical team member prior to your appointment.

Please be aware that the fire alarm will be tested at the surgery every Monday morning at approx. 9am. Unless instructed otherwise by a member of staff no action will be needed by yourselves.
If you are planning on registering with the practice you can submit or download a copy of the NHS registration form via "New Patients". It is worth saying that we do have a defined catchment area and you will still need to attend the surgery for this form to be signed. All new patients will also need to confirm that they are happy to "share" their care records. If you do not wish this then please access the "Summary Care Record" under further information for the Opt Out Form.
By accessing "travel vaccinations" in Further Information you can complete the practice travel questionnaire. This will save you having to collect one from the surgery. Once completed please ensure that you submit and we will print if off this end. Once we have your details we will contact you to arrange an appointment for your travel injections (if needed).
By accessing "prescriptions" you can complete an automated repeat prescription request form which once submitted will be processed by our reception/dispensing team ready for collection within 2 working days (not counting weekends please).
We would also request that you tell us exactly what you need rather than "all my tablets". If we are not sure we will have to contact you for clarification which could delay you getting your prescription.
If you would like to give us some feedback about anything to do with the practice (good or bad) this is a new facility within our website to do so. We do not currently have a Patient Participation Group as we have always felt this is a little limited having a small group of patients representing 6,500.
This is not the right place to discuss issues of a personal nature, these should be raised directly with the practice.
We do have two patient representatives who go to the Patient Congress events
Mr Allan Owen and Mrs Joan Bennett, they submit their comments and responses to the website following these meetings. All patients are welcome to attend.
If you do not have the time to attend meetings but still want to play a part in how the surgery makes decisions then please consider signing up to the "virtual patient group". Ideally you would need access to a computer with internet connection but this is not vital as we can issue hard copies of the surveys on request. See "sign up for our Patient Group situated to the right of this column.
Contact the Patient Group with your feedback, suggestion, comment or question. Please do not use
this form for complaints - these should always be directed to the Practice Manager

be aware that we cannot offer any guarantees of absolute privacy. If this matter
concerns you then you should use another method to notify us of your comment.
Your IP address (126.96.36.199) will be sent
with your communication. In rare cases where abuse or criminal activity can be shown
to have taken place this may be used by the authorities to trace you.
